Achievable Health is a nonprofit whose mission is to provide high quality, integrated health care to individuals with developmental disabilities (such as autism and Down syndrome), their families, and other vulnerable populations. The Achievable Health Center, co-located with one of California’s 21 regional centers, is a first of its kind state-licensed, federally qualified health center which opened in 2013 to meet the healthcare needs of these patients. We provide a full range of health care services, from primary care for adults and children, to mental health services, and access to a vast specialty care referral network.
At our health center, all patients and families have access to extensive care coordination, enrollment and eligibility services, and extended visit times. All of these services are vital for individuals with intellectual and developmental disabilities, and other medically vulnerable populations.
