We are a mobile qualified team of Nurse Practitioners, allied health and geriatrician’s providing support and services to the consumer at the Residential Aged Care Facilities. We provide education and clinical competencies to the Registered Nurses and case conferences to the consumers and families.
We provide bulk billed service and consultations for older person in their home or at the Residential Aged care Facility. The Nurse Practitioner works collaboratively with the General Practitioner in ongoing care of the consumers.
We also provide support for the providers in compliance matters and offer a nursing "boost" to get care evaluations reviewed, updated and support its implementation.
We have partnered with several Sydney based providers to support them audit reviews and help them ahead of any unannounced audit reviews. A simple service that supports their obligations and provides additional comfort for the consumers.
