Actualize Health is an accountable care organization (ACO) focused on expanding value-based care for underserved patients and the practices that care for them.
For years, value-based arrangements have focused heavily on Medicare and Commercial patient populations. We believe underserved patients and their physicians deserve the same level of attention and support to improve outcomes.
Additionally, fee-for-service payment from Medicaid and other government programs are not enough to sustain the safety net in our communities. Practices caring for underserved patients need value-based payment models to remain viable and maintain critical access to care.
