Capital Primary & Geriatric Care, LLC is a physician-owned medical practice currently establishing wound care programs in nursing facilities. The hallmarks of our wound care practice include:
1) A wound care certified geriatrician that provides care onsite in the facility. Dr. Summit Gupta, MD, FAPWHc is the founding physician for Capital Primary & Geriatric Care. He is board-certified in internal and geriatric medicine with wound care certification through the National Alliance of Wound Care & Ostomy (NAWCO) and the Academy of Physicians in Wound Healing (APWH). Dr. Gupta has over a decade of service to nursing facilities, hospitals, and ambulatory wound care centers. He implements a multidisciplinary approach to preventing decubitus (pressure) ulcers and other wounds as well as avoiding F-tags (686) and deficiencies related to wound care. Dr. Gupta’s expertise includes the following advanced wound care modalities: diathermy and electrical stimulation (E-Stim) machines, wound vacuums, multi-layer compression therapy (Coban, Unna’s boot), and skin substitutes (Oasis, ACell). His experience in the wound care center allows him to bring that high level of care to the facility obviating the need to send residents to these clinics.
2) Nurses, trained in wound care, to facilitate wound rounds and promote wound care treatment. Our nurses enter wound care treatment ordered by Dr. Gupta and follow up on these orders by procuring dressings, DME, and other treatments. These nurses are recruited local to the facilities to help Dr. Gupta serve the residents.
3) Advanced wound care health information technology (IT) solutions that include image-based wound care EMR, platforms that track wound healing over time, Continuity of Care Documentation (CCD), and telehealth software.
CP&GC can also expand into general geriatric care for your residents where we can become their primary geriatric providers and similarly implement our nursing support and health IT solutions.
