Our communities affect almost every aspect our health. Although healthcare access in urban areas is on the rise, access to care in rural areas continues to be limited. The Community Health Development Foundation (CHDF) – partner non-profit of Community Health Development Partners – was created to help close the gaps in care experienced by rural and other underserved communities.
Patient education is one of the most important ways to improve the health of communities and because of this, CHDF partners with local healthcare providers, wellness and fitness experts to provide free resources and educational classes specific to the needs of community members.
The healthcare system is complicated and it can be difficult to manage ones health amidst the complexities. CHDF provides the support of a Patient Navigator to guide patients through the healthcare system. This can include recommendations of specific screenings, required labs and prescriptions, as well as communication with their healthcare providers to ensure they understand all the information they need to make an informed decision about their care.
