Currently in the UK, the NHS remains the first port of call for all health issues, but it can't keep up with demand. Resources exist in local communities, but many are not embedded enough to know. That's where Community Bridges come in.
We want to make it easy for people to get to know their community and find a range of local organisations offering accessible services that support their health and wellbeing.
We are working to transform the health & social care sector by supporting:
1. organisations ➡️ by building their capacity to offer accessible health & wellbeing solutions to their community and to be found by those who can benefit from their services
2. people ➡️ by building their knowledge and confidence in leaning on these organisations to tackle isolation and wellbeing issues.
