Compass works with health plans, employers, clinically integrated providers and "payviders" (providers at risk) to help improve care and manage the costs associated with Chronic Kidney Disease (CKD), including End Stage Renal Disease (ESRD). The foundation of our comprehensive specialty management solution is a proprietary insights platform that relies on a multidisciplinary approach to mine available data to recognize clinical risk by identifying CKD early, delaying progression, promoting the most cost-effective alternative of transplantation, and preparing those patients who must start dialysis for outpatient initiation and home therapies. Not only do we strive to improve the quality and cost effectiveness of care for patients diagnosed with CKD, we also seek to detect patients at high risk for developing CKD as well as those we suspect may have undiagnosed disease. Many disease management programs struggle to deliver on the marketing hype. Compass is different.
