We provide skilled, medically supervised services to patients in the comfort of their own home. We offer physical therapy, skilled nursing, and medication management/reconciliation, along with a full coordination of care. We pride ourselves on caring for the whole patient. Our Office Manager, Yadira; Care Coordinator, Yazmin; Office Intake Coordinator, Amanda; Quality Assurance Director, Iosef; Owners Jillian & Paul, and our Director of Nursing, along with an entire team of skilled professionals in the field, ensure a complete coordination of care. Our bridge of care helps to prevent readmission within the critical 30 days of discharge through the management of skilled services and program designed for our client's and their family's unique needs. By bridging the gap between the hospital, skilled nursing facility, and home, we provide a pathway to recovery, renewed independence, and patient empowerment, which create an improved quality of life and overall health. Healthcare can be complicated. Our mission is simple, we advocate for the high-quality care of our patients and their families. Our Medical Social Workers provide assistance during this time of transition. These skilled professionals offer socio-emotional benefits to our families through education and access to available resources, which support the physical, social, emotional, and financial adjustment. Some of the community programs we offer include coordination of a homemaker, caregiver and 'Meals on Wheels.' We also provide assistance obtaining and coordinating transportation, housing, and public aid. We are always looking for talented staff members to join our team and are accepting new patients.
