Het FACT Jeugdteam kan ze helpen om hun leven weer op gang te krijgen. Samen kijken we hoe we dat vorm kunnen geven. Als ze willen betrekken we ook hun omgeving. Het FACT team komt in beeld als andere hulp niet genoeg helpt.
