Horizon Recuperative Care is a 47-bed Recuperative Care Center.
Our mission is to end the cycle of homelessness by providing quality care in a safe and comfortable environment for those who need further support.
Horizon is proud to offer a range of services. We ensure residents are housed, recover from post-acute care, take medication, and attend all follow-up appointments. At the same time, we help residents obtain long-term housing and set them up for success.

Horizon’s program operates within a CCLD Licensed Facility and focuses its services on coordinated planning, coordinated care, and a safe environment.
Horizon’s goal is to transform health services and health outcomes for vulnerable homeless individuals who require a shoulder to lean on as they strive for a better tomorrow.
