POST ACUTE CARE PROVIDER IN CONNECTICUT

REDUCTION OF AVOIDABLE EMERGENCY ROOM TRANSFERS
Collaboration with physician prior to transfer to emergency room.
Educate Primary care providers of ability to directly admit to skilled nursing facility within 30 days of discharge without requirement of emergency room visit.
We provide medications upon discharge from facility to cover until next primary care appointment.
Communication with community primary care providers prior to discharge from nursing home.
