Journey Home Hospice - a program of the Saint Elizabeth Foundation - provides 24/7 residential hospice palliative care for patients and their loved ones experiencing homelessness and structural vulnerability. Currently, the hospice has thirteen beds in downtown Toronto and three beds in Windsor. We believe in equitable access to care and our interdisciplinary team of care providers and volunteers offers specialized trauma-informed, culturally safe, and harm-reducing approaches to care with individual developed care plans. As an international teaching and learning organization, we are dedicated to advancing capacity in the delivery of specialized care and regularly share our knowledge with visitors, empower other organizations across Canada and beyond, and exchange knowledge through conference and professional educational opportunities.
