Lizzie's Consultant Pharmacist help individuals and caregivers help their ill relatives by being there to help them understand medical terms. It's a challenge to know what's going on if you don't understand the words being used to communicate by the health professional. Our mission is to help fix health concerns by collaborating with patients and/or healthcare providers. Services offered included medication appropriateness, medication consultations, Comprehensive Medical Review, clarifying the use of medication therapy, disease state monitoring, and improvement. Coaching will educate patients to better self manage health. Our vision long-term is to advocate for self-awareness in those individuals with health conditions poorly managed and for caretakers. Ideas we have, getting them back to a better functioning state with close medication monitoring and nutrition adjustments, and doctors visit when appropriate.
