Mentor Clinic is a demonstration clinic for the “health home” model of primary care. It is intended to be like the school system – if you live in this neighbourhood, you come to this clinic for care. You are attached to the clinic, not to an individual healthcare provider, so that if providers go on vacation, retire, leave or are unavailable, you will still get your healthcare services at this clinic.
