Mint Health NY is a group of medical providers who travel to patient's homes or do tele-health visits to certify them for home health services. Many elderly people need home-health care such as a home health aide, a skilled nurse, or physical therapy, but can not get those services without a note from a doctor. However people may not have a primary care doctor to go to or can not leave the home to get to a doctor to get this order completed. That's where mint health comes in. Our doctors go to the patient's home so they can put the order in for them and to get the home health services started. We partner with home health agencies by visiting the patients and signing orders for those who can not make it to their Primary Care Physician or who do not have one to sign home care orders. We also work on behalf of individual clients who are looking to receive Home Health services but need an agency to deliver the services. In addition to home care certification, we complete regular annual check ups, fulfill prescriptions, order DME supples, and much more.
