North West Hospice provides palliative care services for people with life limiting illnesses where they are residing, be that at home, in a nursing home, hospital or in our hospice. Palliative care involves meeting an individual and assessing their needs and those of their families , spiritually, physically , socially and emotionally. These needs are then addressed with the aim of improving quality of life for the patients and their family. In order to provide this care, North West hospice has a team of professionals including Doctors, Nurses, Social Workers, physiotherapists, occupational therapists and chaplaincy services.
Our Inpatient Unit has eight beds (three single rooms and a five-bed area) where people are cared for by a multi-disciplinary team and an extensive range of volunteer services for a variety of reasons, including symptom control and end-of-life care.
Our Community Palliative Care Team visit patients in their own homes, community hospitals or nursing homes. This team of clinical nurse specialists, social workers and doctors work very closely with the public health nurse and the patient’s GP.
Our Hospital Palliative Care Team provide support to Sligo University Hospital’s medical and nursing teams and palliative care to patients and their families. They aim to provide a seamless link in the flow of care between the hospital, the hospice In-Patient Unit and Community Palliative Care, assisting patients and their families to make the most appropriate decisions according to their individual care needs.
