Northeast Practitioner Management, Inc Overview

Northeast Ohio Practitioner is a specialty NP program focusing on appropriate management of avoidable readmission's to the acute setting from post acute providers. The specialty program does not interfere with Physician visits or their NP's in those settings. The service is provided to most post acute providers and requires no stipend or Directorship from the partner post acute providers. Our program focuses on training, management and company protocols for Nurse Practitioners to reduce avoidable readmission's to the Acute Care setting. The program is currently managing a 5% return to acute with the providers that have been on service for 60 days or longer. As we are a Northeast Ohio based organization we do work with post acute providers nationally and are operating in multiple states by the end of 2018.
