Transitional Care
NP Housecalls supports you and/or your loved one with comprehensive primary care after major surgery, hospitalization, skilled nursing facility admission or emergency room visit. Comprehensive transitional care with NP Housecalls begins first with in-depth, post-discharge home visits where a Nurse Practitioner works with you and your family to customize a health plan specific to your recovery and long-term health goals. NP Housecalls collaborates proactively with your surgeon, specialist and/or primary care provider, your family and/or residence facility staff to ensure that you receive the collaborative care and resources requisite for your optimal health outcome.
Palliative Care
Palliative care is a multidisciplinary approach to specialized medical and nursing care for people living with chronic conditions and/or a life-limiting illness. NP Housecalls’ experienced and compassionate team of Nurse Practitioners focus on providing relief from the symptoms, pain, physical stress, and mental stress at any stage of illness with the goal of improving quality of life.
