PCC’s mission is to Advocate, Assess, Monitor and Connect participants to the most appropriate community services that enable and maintain their health, safety and independence in the community. PCC focuses on ensuring individualized services for participants through maintaining participant-centered services with an interdisciplinary team approach. PCC provides services under Community HealthChoices, Nursing Home Transition and the Office of Long Term Living and continues to provide services to participants under the OBRA Waiver and Act 150 Program as well.
