Pragmatic Solutions Ltd. is a privately owned company, specializing in safety and disability management, delivering on the opportunity for better workplace safety by way of Information Technology. The challenge of implementing software solutions led us to discover that current safety and loss management systems are dysfunctional and that it is not possible to implement software without first rectifying the underlying systemic problems. For the last decade we have been operating as a private safety think tank investing considerable resources into research and design, investigating the reasons for failed safety and developing practical solutions. A key fork in the road for us was making the decision to provide industry with what they needed rather than what they wanted. There is a wide gap between the level of knowledge published and debated in the academic circles on what is known to work and the level of knowledge transfer that has occurred from these circles to the industry or regulatory circles. This should not be dismissed as ivory tower and theoretical debates. There are serious consequences for the way we go about incident prevention. This has led us to adopt the following quote as a tag line: It ain’t what you don’t know that gets you into trouble.
