With over 15 years of direct support to our communities, we specialize in providing care in physical, occupational, and speech therapy in private homes, senior living, and retirement communities.
Through our years of one-on-one care, we have become a dedicated and trusted voice in our communities and a source for support. Our passionate team of clinicians provides professional and respectful care, based around our Continuum of Care Program. Offering our patients, a flexible, personalized treatment plan.
We believe in supporting the kind of life that you or your loved one wants to lead, in settings where they feel comfortable and empowered. That’s why we created our ‘Fall Prevention Program,’ to provide a holistic assessment of the patient, their surroundings, and what simple things we can change in one’s living area to help prevent falls.
Our physical therapists can then help with muscle strengthening, balance, and gait analysis to identify specific muscular and postural deficits that may lead to falling. Then, we develop and implement a personalized program for maximal rehabilitation.
Our occupational therapists specialize in geriatric and dementia care. Working with compassion, our team uses adaptive tools to help challenge and build confidence, while implementing strategies that reduce anxiety in our patients.
We know that you are entrusting us with helping your loved one. That’s why we believe in honest communication as the cornerstone of what we do. We have integrated, structured calls and are happy to go over any specifics, changes, or challenges every step of the way.
Currently, our team of therapists is helping in the Delaware, Chester, Montgomery, Berks & Lancaster Counties of Pennsylvania. But we are in the process of expanding our network of partners and communities served. Check back again soon!
