Samen Kwiek is een online platform dat de link legt tussen welzijn en zorg. Op het platform staat al het aanbod in Lingewaard op het gebied van welzijn, zorg, gezonde leefstijl en sport. Denk aan fysiotherapie, maar ook aan wandelclubs of biljartmiddagen. Met Samen Kwiek kunnen welzijns- en zorgprofessionals uit de nulde, eerste, tweede en derde lijn makkelijk inzetten op preventie. Zij kunnen inwoners, cliënten en patiënten eenvoudig doorverwijzen naar een activiteit of zorg die bij ze past, en waar ze zich blij en gezonder door voelen. Onze uitgangspunten zijn de principes van Positieve Gezondheid. Met het platform voorkomen we onnodige (ziekenhuis)zorg en sluiten we aan op het Integraal Zorgakkoord en de steeds digitaler wordende wereld. Samen Kwiek is een samenwerking van Gemeente Lingewaard, Rijnstate, Stichting Welzijn Lingewaard, Health Valley Netherlands en Pact Care.
