The San Diego Wellness Collaborative is a nonprofit organization that improves health equity and population health in San Diego through multi-stakeholder collaborative initiatives. Our initiatives can be viewed along a continuum, from “upstream” prevention to “downstream” clinical care. Effective treatment, addressing social needs, and improving community conditions all play a role in achieving health equity. SDWC coordinates efforts across this continuum to support community health. Our initiatives include San Diego Accountable Community for Health, Neighborhood Networks, and Be There San Diego.
