We partner with community-based organizations by embedding our comprehensive health services to increase health access, equity and quality, especially in under-resourced communities of color. Our innovative reverse integrated care model leverages a truly multidisciplinary & culturally-diverse team of physicians, nurse practitioners, behavioral health providers, care managers, and family navigators to achieve optimal health & well-being for the communities that we serve.
