A unique skill set and innovative thinking led to a mid-career epiphany when his increasingly frequent encounters with young women with gastroparesis who were being considered for endoscopic pyloromyotomy allowed him to time to appreciate the pattern of extraintestinal symptoms as part of broader syndromes, including joint pain, widespread autonomic dysfunction, and allergic-type symptoms. The common thread has irreversibly changed his career. "Functional" GI disorders are so often downcast by gastroenterologists that it has become normative. Yet all along these patients offered a window to the complex, (yet more intuitive) connections between multiple organ systems. On closer inspection, seemingly disparate symptoms could have been traced like ripples through immunologic, neurologic, psychiatric, and structural dimensions, in each and every patient. In the classical medical model, these symptoms have long been inexplicable and this has led to derogatory pseudo-psychiatric, or psychosomatic diagnoses. We haven't come so far from "hysteria," afterall. The prime example is the stubbornly 'rare' connective tissue disorder, Ehlers-Danlos Syndrome, which is his current focus. Observation and study of these patients and his increasing knowledge of the immunologic and neurologic components and their management strategies has separated Penn from his partners and colleagues. Many simply aren't interested. Others rebel or refuse to consider it. This is not without precedent in medical history: H. pylori, Celiac disease, Eosinophilic Esophagitis. There is no going back for Penn, any more than un-learning how to ride a bike, or to read. Dr. Penn is determined to raise awareness for EDS and the lessons it has for the interactions between multiple systems that are likely present in more common conditions.
