Why we started
Our founders witnessed the human cost of avoidable readmissions when a family friend cycled in and out of the hospital. Each readmission felt preventable with better data visibility, yet the care team lacked timely signals. We began by tackling cardiac readmissions, a high-penalty problem, but designed our architecture for any condition that benefits from early warning.
