Connection SGGZ is een erkende zorginstelling in de geestelijke gezondheidszorg. Wij zijn als zorginstelling toegelaten door het Ministerie van Volksgezondheid en Sport en zijn in het bezit van een WTZa-vergunning. Wij bieden Specialistische Geestelijke Gezondheidszorg (SGGZ) en specialiseren ons in het behandelen van verslaving en onderliggende problematiek.
Ons aanbod is gericht op mensen vanaf 18 jaar die te maken hebben met een verslaving. Naast de focus op het aanpakken van de verslaving is er dus ook veel aandacht voor onderliggende problematiek die de verslaving mogelijk mede in stand houdt. Denk hierbij aan angst- en stemmingsstoornissen, trauma-, gedrags- en/of persoonlijkheidsproblematiek.
Connection SGGZ behandelt volgens bewezen effectieve behandelmethodes, ook wel “evidence-based” genoemd. Dit wil zeggen dat wij volgens de richtlijnen van meetbare kwaliteitsnormen behandelen. Wij weten dat ieder mens anders is en daarom is bij ons de cliënt leidend en niet de behandeling. Op een persoonlijke en betrokken wijze kijken we samen met de cliënt (en met eventuele naasten) welke behandeling het beste past. Afhankelijk van de zorgvraag van de cliënt wordt er een behandelplan op maat gemaakt, waarna de behandeling gestart kan worden.
Ons multidisciplinair team bestaat uitsluitend uit specialisten, professionals en ervaringsdeskundige zorgverleners die elkaar goed aanvullen. Hierdoor ontstaat er synergie en zijn wij in staat om onze cliënten op professionele en persoonlijke wijze een kwalitatief hoogwaardige behandeling te bieden.
Om duurzaam herstel te realiseren is een herstelgerichte werkwijze noodzakelijk. Naast de professionele interne inrichting werkt Connection SGGZ daarom samen met verschillende professionele ketenpartners.
Wij zijn als zorginstelling positief ingesteld en zorgen er samen met de cliënt in een gastvrije sfeer voor dat mensen weer in hun kracht komen te staan, zodat duurzaam herstel mogelijk is.
