La Fondation Cap Diamant est un organisme sans but lucratif qui administre des programmes de transfert de fonds destinés à soutenir financièrement les personnes aînées démunies et isolées à demeurer dans leur domicile le plus longtemps possible. Outre l’aide domestique apportée à la personne qui en a besoin, l’aide apportée par la Fondation permet à cette personne de bénéficier d’une présence sur une base régulière à la semaine ou aux deux semaines. Le préposé ou la préposée peut donc constater si la personne s’alimente bien, ne souffre pas de dénutrition, n’est pas abusée par un proche ou encore si elle n’a pas de problèmes de santé liés par exemple à une chute ou à l’émergence d’une perte cognitive.
