MedMatch Network is a cloud-based, AI-enabled, SaaS platform providing an end-to-end solution for provider-patient engagement while empowering patients with access to unified health data and a marketplace of digital health applications to improve health care access, delivery, and equity.
The open registration platform invites providers to create and verify their credentials and publish their practice profile and availability. Providers can connect with existing colleagues or engage with new ones to create their own vetted network. A patient application interface, MedMatchNet, integrates with the provider platform to provide patient engagement and also provides patients with access to their provides network, scheduling, and a gateway to digital health.
Integration with electronic health records ensures interoperability, portability and quality referrals. The platform FHIR-enabled database securely hosts patient information and makes it accessible when needed. The tracking feature informs on patient referral status and updates everyone involved through confirmations and reminders thereby preventing referral leakage.
