MindTheGap biedt één op één begeleiding aan voor kinderen en jongeren die in een pleeggezin of gezinshuis wonen. De leeftijd varieert tussen de 4 en 18+. MindTheGap kan hulp bieden als de pleegouder/gezinshuisouder er niet uitkomt met de jongere of het kind. Ook kan de hulp worden geboden als de gezinshuis-/pleegouder geen tijd of ruimte heeft voor de hulpvraag of ter ontlasting voor de gezinshuis-/pleegouders.

De begeleiding wordt uitgevoerd door Lisanne. Zij is al bijna 10 jaar een 'eigen kind' in een gezinshuis. Hierdoor maakt zij van dichtbij mee, hoe het is om met deze jongeren en kinderen te werken en heeft zij hier een passie voor ontwikkeld.
