Το τμήμα μπάσκετ του Πανιωνίου ιδρύθηκε το 1919 στη Σμύρνη και ήταν από τα πρώτα στον Ελλαδικό χώρο.
Το 1918-1919, όταν ακόμα ο Πανιώνιος βρισκόταν στη Σμύρνη, πρωτοστατεί στη διάδοση του μπάσκετ. Το Μάρτιο του 1919 ο πρόεδρος Δημήτριος Δάλλας ιδρύει τμήμα μπάσκετ, στο οποίο ως τα τέλη του έτους μετείχαν ενεργά 78 αθλητές.
Από τα μέσα της δεκαετίας του ’70 με πρωτοβουλία του Ανδρέα Βαρίκα δημιουργήθηκαν τμήματα υποδομής και αναδείχθηκε μια νέα φουρνιά καλαθοσφαιριστών που ανέβασαν τον Πανιώνιο στην Α΄ Εθνική, όπου πρωταγωνίστησε.
Από το 1984 και επί μία δεκαετία ήταν μόνιμα στην πρώτη τετράδα. Η μεγάλη στιγμή του Πανιωνίου ήρθε το 1991, κερδίζοντας τον ΠΑΟΚ στον τελικό κυπέλλου.
