La Plateforme ETP29, subventionnée par l’ARS Bretagne est un dispositif de soutien à la promotion et au développement de l’Education Thérapeutique du Patient (ETP) sur le territoire de démocratie en santé
« Finistère Penn Ar Bed ».
Structurée sous forme d’association, notre principale mission est l’accompagnement des équipes de soins (privées, publiques, de premiers recours) dans la gestion de projet et la méthodologie en ETP.
Activités spécifiques :
- Recensement des programmes ETP du territoire
- Pour chaque programme, création et mise à disposition en ligne d’une fiche synthétique descriptive
- Soutien à la construction d’un parcours éducatif
- Aide à la co-écriture d’un dossier déclaratif
- Accompagnement à répondre à un appel à projets
- Elaboration d’un rétroplanning
- Mise en lien avec d’autres équipes/acteurs ETP (patients, professionnels)
- Apport de ressources bibliographiques en ETP
- Orientation vers des formations en ETP
- Soutien à la communication en ETP
