Het Nationaal Programma Palliatieve Zorg II (NPPZ II) staat voor de integratie van palliatieve zorg in de reguliere zorg, via de implementatie van het Kwaliteitskader Palliatieve Zorg. Dit is in lijn met de doelstellingen van het Integraal Zorgakkoord en het doel van het NPPZ II: voor iedere patiënt en naasten de juiste zorg, op de juiste plek, op het juiste moment, door de juiste zorgverlener, met de juiste financiering.
Elk jaar verkeren 120.000 mensen door een ongeneeslijke ziekte of ouderdom in de laatste fase van hun leven. Door de vergrijzing stijgt dat aantal enorm. Al die mensen hebben palliatieve zorg nodig. Zorg waarin niet langer genezing, maar kwaliteit van leven, van rouwen en van sterven centraal staat. Daarom maakt het NPPZ II zorgverleners bekend en vertrouwd met palliatieve zorg en ondersteunt hen daarin. Dat doen we samen met onze partners, met veel aandacht voor patiënten en hun naasten. We richten ons op het algemeen publiek om duidelijk te maken wat palliatieve zorg voor mensen kan betekenen. Op het volledig integreren van palliatieve zorg in de reguliere gezondheidszorg via het kwaliteitskader palliatieve zorg. En op het verbeteren van de samenwerking tussen alle betrokken zorgverleners.
