Samenzorg Netwerk is een domeinoverstijgend samenwerkingsverband in de regio Noord- en Midden-Drenthe. We werken met vijftien partners in het regionale (zorg)netwerk samen aan verbetering van de ketenzorg. Samen organiseren wij zorg en ondersteuning, waarbij we met en voor kwetsbare inwoners streven naar een optimale kwaliteit van leven. Met de nadruk op veerkracht en eigen regie.
De kracht van ons netwerk zit in de regionale samenwerking, die op een heel natuurlijke manier tot stand komt. Mensen kennen elkaar, vertrouwen elkaar en zijn gedreven om samen de zorg goed en efficiënt te organiseren. Met steeds meer aandacht voor gezondheid en het voorkómen van zorg.
We zijn in 2018 met elf partners gestart als Samen Zorg Assen en gaan sinds de zomer van 2024 verder als Samenzorg Netwerk.
