ELFT lead on daily multi-disciplinary review of people in acute beds who no longer meet the clinical criteria to require inpatient care and can be discharged to a non-acute setting Working with brokerage to source alternative non acute residence where required. Documenting clear clinical data on EMIS; AZEUS; SystemOne software for discharge that can be enacted by appropriate clinician Manage coordinated discharges from hospital enabling rapid discharges within 3 hours for medically optimised patients Coordinating and implementing appropriate healthcare, social care or community therapy needs as required. Liaising with other HUBs Utilise discharge pathways as a discharge route from acute to community rehabilitation bedsAct as trusted assessor for onward referrals using the IDH referral form Maintaining daily audits data Reporting / escalating issues to Operational Manager; CCG where appropriate Provide leadership in the implementation of the national framework for continuing healthcare and NHS funded nurse care, including care for the learning disability service
