➢ I worked as a Coordinated Entry Specialist for the Eastern Pennsylvania Continuum of Care at Housing Transitions. I assessed individuals and families experiencing homelessness or at risk of homelessness by phone or in-person. At HTI, I assisted 𝟱𝟬𝟬+ households experiencing homelessness or housing insecurities. ➢ I then made referrals to connect them with service providers that can best meet their needs in stride to obtain and uphold permanent stable housing.➢ I was the first point of contact for those experiencing a housing crisis. I utilized an all-encompassing approach to a crisis response to meet the needs of the individual(s) in a swift and timely manner.
