Managing multiple projects for the Hampshire Health Record (HHR) programme:• Explore options to replace the current Electronic Palliative Care Coordination Systems (EPACCS) system with an improved, more functional and more widely available shared End of Life (EOL) care plan service for Hampshire palliative care patients using HHR.• Hampshire GPs have a directed enhanced service (DES) requirement to generate care plans for 2% of their registered population. These are generated on the GP clinical system and output in paper format before being faxed to GP out of hour’s service and the local ambulance service. The project will enable HHR to replace this paper/fax process and to provide a safe and effective way of maintaining and sharing these plans with appropriate out of hours services. In order to achieve this and provide a safe and effective service, developments are required to HHR to enable functionality to upload admission avoidance care plans (AACPs) to HHR.• Plan future developments on HHR which will enable auto creation of EOL and AACP records so health professionals do not have to re-create these care plans on HHR.• Plan future developments on HHR which will enable auto creation of EOL and AACP records so health professionals do not have to re-create these care plans on HHR.• Proof of Concept for a patient portal into HHR using Microsoft HealthVault verification software. Responsible for the project management, training, engagement, administration and support of Florence simple telehealth texting service as part of the AIM for Health project being conducted across Portsmouth, Fareham and Gosport and South Eastern CCGs. They will encourage individual practices to recruit appropriate patients for two or more of the selected clinical protocols.The focus of the role is the delivery of telehealth for patients with specific conditions outlined at the beginning of the project but which may include asthma, hypertension, smoking cessation, medication reminders and the provision of text relayed preventative health care to these patients; as well as the monitoring of effectiveness in terms of clinical outcomes and healthcare resource use –in recruited patients. As an autonomous practitioner I am responsible for the practice application in line with the AIM for Health specifications for the project that have been endorsed by the CCGs.
