· Utilize harm reduction interventions and methodologies to provideclient centered services to keep them in housing.· Practice Trauma Informed Care and Motivational Interviewing.· Assist residents to build upon their strengths and recognize their needsin a respectful manner· Work collaboratively with other community support providers,outreach teams, Housing Authorities (Housing Specialists), SocialWorkers, Leasing Team, Landlords.· Keep accurate, up to date documentation on all clients, including:individual housing plans, emergency plans, case notes,case file, assessments, call logs, Housing Authority· Be knowledgeable of community resources and refer clients asneeded.

· Screen calls from referring Outreach partners, PD and returningclients.· Ensure HMIS intake forms are completed by clients and data isentered into the HMIS.· Maintain accurate records of client enrollments and progress notes.· Provide employment linkage, benefits establishment, linkage tocommunity providers for substance abuse, primary and mental healthcare, and all other services needed to assist clients in reaching theirstabilization goals.· Conduct crisis and risk assessments in consultation with supervisor andcase management team· Provide crisis intervention services focused on enhancing the client's’ability to independently problem solve, utilize effective coping skills,and manage and self -coordinate own care
