Representing William Osler Health System, I've had the privilege of leading provincial and national Quality Improvement projects in the implementation of senior-friend hospital initiatives. My efforts have been recognized with two prestigious Interprofessional Clinical Awards of Excellence in the categories of Early Career and Clinical Leadership.Currently, I serve as a Transition Care Leader with SE Health, where I'm deeply involved in implementing an innovative care model aimed at reducing ALC (Alternate Level of Care) days, breaking down traditional care barriers, and preventing readmissions among our vulnerable population. We are trailblazing and disrupting traditional models of care to keep patients at home longer, and healthier.I'm excited about exploring new opportunities and leveraging my leadership skills to contribute to boards or councils in a volunteer capacity.
