Clinical team leader in a new innovative home healthcare delivery model as a preamble to the Ontario Health Teams (OHT) and independent of Local Health Integrated Network (LHIN). Using QI processes and understanding the gaps, opportunities, and evidence in clinical care, I have a focus to develop impactful and innovative volunteer services to support service excellence and patient experience.Managing a small team of professional volunteer administrators and 1,200 volunteers across all three sites at Osler required expert team building skills and innovative use of technologies to integrate and enhance communication. Utilizing proven managerial, coaching, and people engagement techniques from Studor Coaching, CCHL's LEADS Framework, and tracking vital metrics, appreciation and enjoyment of work has increased significantly.As a duly registered Occupational Therapist, this position aligns closely with vocational skill acquisition for students and adults, while leveraging and optimizing occupational transition for retirees to contribute back into the community in a meaningful matter to increase aspects of physical, cognitive, emotional, social and spiritual self.

Brampton Civic Hospital

Working together with the Geriatric Clinical Nurse Specialist, I am tasked with the program planning, implementation, expansion, and program evaluation of the Hospital Elder Life Program at Brampton Civic and Etobicoke General Hospital. The HELP model is an evidence-based program utilizing specially trained volunteers to implement protocols that specifically targets 6+ geriatric risk factors to prevent delirium and functional decline. Clinically, I am in charge of performing comprehensive geriatric assessments, daily screening, enrollment, and re-evaluation of patients into the program while working collaboratively with the inter-disciplinary team to provide consultation on complex behavioural and psychological symptoms of dementia. I am in charge of training, supervision, support and coordinate volunteers to arrange optimal patient coverage. Secondarily, I am the NICHE Coordinator (Nurses Improving Care of Healthsystem Elders) for the EGH campus tasked to oversight the implementation of geriatric quality improvement initiatives in order to push the NICHE Accreditation to Exemplary status. This includes 1) creating a multi-modal training resources for nursing and allied-health using principles of adult education. 2) integration of system services that was traditionally working in silos. 3) piloting new geriatric consultation model at William Osler. 4) meet with all stakeholders for collaboration. 5) complete corporate wide geriatric survey to complete gap analysis. 6) expansion of HELP program to corporate-wide. 7) create local and national partnership and create leadership opportunities. 8) consult and review several order sets and quality based procedures (QBP)My third portfolio acts as team lead in two geriatric quality improvement initiatives. Brampton Civic Hospital

Cardiology and ICUAssess and provide early interventions for patients in Cardiology, CCU and ICU. Collaborate in interprofessional team for successful discharge planning. Team lead in various corporate projects including the draft protocol for a cognitive stimulation protocol for patients in ICU; team lead in the development of Quality Based Procedure (QBP) protocol for Congestive Heart Failure for Occupational Therapy and Physiotherapy; and founder and co-chair for OT Academic Journal Club.Special interest in geriatric care and functional-cognitive assessments. Short Term Assess and Restore Geriatric RehabilitationProvide assessments, collaboratively design individualized treatment goals in order to maximize functional independence for returning home or alternative placement within 2 weeks.

Provide coverage for all inpatient units across three hospital sites; including ICU, medicine, surgery, rehab, complex continuing care, and mental health. My main role was to provide seating, cognitive, perceptual, physical, and functional ADL assessments; develop and supervise a rehab program for OTA to deliver; and collaborate with multi-disciplinary team for discharge planning.

Toronto General & Toronto Western Hospital

Provide float coverage to internal medicine, cardiology, neurosurgery/spinal, and orthopedic. Conduct various cognitive/perceptual, ADL, mobility, and seating assessments to facilitate discharge planning. Educate both patient and patient's family in optimizing independence or inter-dependence to reduce caregiver burnout.Also participated in OT Month Promotion Committee and act as HELP Ambassador at TWH.

Pilot Medical Short Stay Unit (<48 hours medicine inpatient stay).Working in collaboration with interprofessional team to conduct safety and ADL assessments, with a large component in providing insight building and education on task modification, work simplification and recommending assistive/safety aids for use at home or make referral to rehab.

Provide in-home OT assessments and recommendations in the area of attendant care / Form 1, housekeeping and home maintenance, care-giving and non-earners benefit. Occasionally, my role involves educating clients on proper body mechanics, energy conservation and task modification strategies in order to facilitate reintegration of client's ADL to their daily routine.
