Lead Elder Life Specialist
Working together with the Geriatric Clinical Nurse Specialist, I am tasked with the program planning, implementation, expansion, and program evaluation of the Hospital Elder Life Program at Brampton Civic and Etobicoke General Hospital. The HELP model is an evidence-based program utilizing specially trained volunteers to implement protocols that specifically targets 6+ geriatric risk factors to prevent delirium and functional decline. Clinically, I am in charge of performing comprehensive geriatric assessments, daily screening, enrollment, and re-evaluation of patients into the program while working collaboratively with the inter-disciplinary team to provide consultation on complex behavioural and psychological symptoms of dementia. I am in charge of training, supervision, support and coordinate volunteers to arrange optimal patient coverage. Secondarily, I am the NICHE Coordinator (Nurses Improving Care of Healthsystem Elders) for the EGH campus tasked to oversight the implementation of geriatric quality improvement initiatives in order to push the NICHE Accreditation to Exemplary status.
