CurrentPharmacotherapy Management:• Designing rational pharmacotherapy plans for critical care, multi-diseased, elderly, paediatric, and pregnant patients based on patient-specific factors and therapeutic goals.• Monitoring and evaluating pharmacotherapy effectiveness and safety2. Medication Safety Review and Management:• Evaluating prescriptions for actual / potential drug interactions, adverse drug reactions (ADRs), and other drug-related problems (DRPs). • Ensuring safest therapeutic alternatives and safety protocols• Reporting adverse drug reactions (ADRs) and drug-related problems (DRPs)• taking proper measures to ensure safe & effective Medication use 3. Patient Counselling and Education:• Counselling patients on medications, disease progression, and potential outcomes• Providing guidance on adherence to pharmacotherapy and nutrition.4. Prescription Handling and Dispensing:• Prescription handling and dispensing medicine as per professional protocols.5. Clinical Rounds and Collaboration:• Participating in ward rounds and collaborating with medical teams to ensure safe pharmacotherapy plans in critical care units (CCU), intensive care units (ITC), emergency rooms (ER), and paediatric wards through discussions and recommendations. 6. Critical Care Pharmacy and Emergency:• Critical Care pharmacist activities including drug selection & dose adjustment based on hemodynamic, anticoagulation, therapeutic response monitoring, emergency response etc. at CCU, ITC and ER • Ensuring medications in crash carts are labelled and managed properly7. Drug Information and Education:• Operating a drug information centre for medical teams and patients• Providing accurate and timely drug information and education
