She has a diverse professional career of over twenty years in both Nursing and Midwifery, with current regular practice in both disciplines. Abigail worked as a Maternity Investigator for the Healthcare Safety Investigation Branch (HSIB), for several years, where she developed advanced skills in interviewing, report writing, liaising, negotiating, and supporting families and staff, following bereavement and serious incidents. She is the clinical investigation lead for the Baby Lifeline healthcare investigation course run at Cranfield University. Abigail designed the investigation case simulation for this course and provides lecturing support. She has worked in hospital, community, and research settings, within women’s, surgical, medical, emergency, mental health, bereavement, and palliative care. She has also held senior clinical nursing and midwifery leadership roles in addition to working overseas for four years as a Registered Midwife. Abigail has completed accredited Expert Witness training with Cardiff University Bond Solon and is working within this field. She also works as an External Clinical Advisor for the Parliamentary and Health Service Ombudsman (PHSO), as well as a Maternity Specialist Advisor for the Care Quality Commission (CQC). Abigail conducts independent patient safety incident investigations & training in Trusts across the UK. She has taught and mentored midwifery/nursing/medical colleagues/students, written guidance, and policy both in the UK and in Canada. As well as operating her own consultancy, Abigail is an Associate of Systemic Factors Limited.
