Improvement Planning and Coaching Research Quality Improvement methods, tools and best practices and develop QI supporting materials. Contribute to developing quality standards, methods and tools for assessing performance against standards Provide education and training on the use of improvement methods and tools that builds staff capacity and foster a culture of continuous quality improvement. Provide coaching and technical assistance to teams conducting improvement projects, including assessing current practice, identifying barriers and opportunities, and developing and implementing action plans. Support adaption of service models and systems to be responsive to a changing environment Data and Measurement Compile data (performance measures, documentation, observation, surveys, interviews) to identify gaps in service delivery and staff competencies. Collaborate with programs, improvement teams, and other agency representatives to develop measurable performance goals and objectives. Identify/adapt/develop metrics for assessing and driving improvements in services quality, safety, client outcomes, and favorable experience of staff and persons served. Collaborate with the Evaluation team on the design, review and analysis of agency, program‐ and improvement project‐level QI reports and dashboards. Management and Strategy Develop work plans that support successful implementation, adherence to timelines, and progress towards goals. Track and report regularly on the status of quality improvement initiatives across the agency and support continuity and accountability. Assist in development of departmental strategies to advance improvement culture and capacity across the agency. Foster strong relationships with agency support departments including IT, HR, Finance and Quality Assurance to strengthen program operations and maximize efficiency

1. Manages primary aspects of quality program for the Health Home Division, monitoring key health outcomes, establishing performance improvement projects, and resolution of gaps-in-care.2. Oversee quarterly audits, results and implement actions to address any identified improvement opportunities.3. Lead monthly QI meetings to ensure compliance and adherence to all metrics, policies and protocols.4. Manage implementation and execution of corrective action plans in response to program evaluation reports5. Identify trends from standardized chart review tool(s) to enhance compliance of care coordination activities with all federal, state, local, and agency regulations.6. Supervise chart audit practices through development and maintenance of appropriate guidance/policy documentation of these processes.7. Synthesize data that aggregates quality projects outcomes (chart reviews, incident reports, gaps-in-care, quality scorecards, HEDIS measures, etc.)8. Supports implementation and update of Federal, State, local, and payer policy/guidelines, inclusive of Health Home policies changes/updates.9. Develop written action plans using PDSA model to improve any deficits in documentation/records.10. Monitors execution of appropriate QI activities (e.g. staff training, coaching) to support quality oversight and ensure that corrective action plans are completed by target dates.11. Monitors quality metric benchmarks such as HEDIS measures, triple aim initiatives (reduce hospital and ER/ED visits; increase primary care connections)12. Oversee reportable incident protocol and review compliance with all incidents reports in accordance with lead health home and and/or New York State Department of Health policies and recommendations.13. Support development and delivery of training/education to improve QA/QI outcomes.14. Attend system-wide quality initiative meetings, and all other out of system quality initiatives/conferences to represent Health Home program.

Oversight and coordination of school based mental health programs in at least 5 Queens schools.Maintain good relations with schools through regular on-site contact with school staff.Collect referrals and intakes from all assigned schools. Follow up on referrals and intakes.Provide on-site individual, family, and group psychotherapy as well as crisis intervention.Oversee therapists in assigned schools by conducting site trainings; ensure therapists are informed of the school policies, procedures, and schedules; assist with patient scheduling and other logistical issues that therapist may have.Provide mental health therapy to children in schools from K-12th grade.
