• Perform weekly comprehensive medication review/medication therapy management using Outcomes MTM and Mirixa for eligible patients to enhance patient care, control healthcare costs, and improve patient results• Establish daily initiatives to control inventory, increase sales, and reduce pharmacy expenditures• Maintain and prioritize workflow to manage an average of 1,200 prescriptions weekly within payroll limitation• Manage pharmacy technicians and interns• Act as a preceptor to pharmacy students during their rotations• Administer vaccines (influenza, pneumonia, Tdap, shingles, etc.) averaging over 500 shots given annually• Dispense prescriptions using ARx and Enterprise pharmacy systems• Educate patients on disease state management along with medication use and side effects• Recommend over-the-counter products according to patient symptoms, drug allergies, medication therapies, and medical history• Compound medications to meet specific needs of patients• Consult with healthcare professionals and 3rd party providers to identify appropriate and cost-effective alternatives for patient care• Collaborate with healthcare professionals in regard to medication dosage, drug interactions, disease management, and potential side effects of prescription and non-prescription medications• Ensure proper dose adjustments for anti-infective therapies based on patient’s age, weight, and diagnosis• Facilitate monthly interventions identified by health plan providers to optimize medication regimens and increase patient awareness and adherence.• Provide daily “caring calls” to patients on anti-infective therapies to ensure symptoms improvement, identify adverse reactions, and resolve any questions or concerns• Monitor patient prescription use history through the Illinois Prescription Monitoring Program prior to dispensing controlled substance prescriptions• Coordinate with insurance providers to manage billing and reimbursement issues
