Discharge Coordinator
CurrentTackling homeless and financial hardship on NHS mental health wards across WiltshireWorking closely with psychiatrists, ward managers and social workers in removing finance / housing related barriers to dischargeSpeeding up recovery rates by removing housing / finance related l barriers to discharge Reducing risk of readmission through multi-agency working with the NHS, social sector and private sector Working closely with social and private landlords in minimising risk of evictionWorking closely with family members in unpicking and resolving housing / finance related stressors Working closely with local authorities in getting homeless service users housedCoordinating referrals and applications for emergency housing, long-term social housing and private tenancies Supporting a large caseload of service users to access the benefit system from the point of admission (so that they have money in the bank upon discharge)Giving specialist support and advice to service users with unmanageable mortgages Providing support, guidance and advice to service users with significant debt (including manageable repayment plans and Breath Space referrals)
