I review new patient histories to create a summary of the progression of the primary disease processes to present the cases to the staff internists. I interview clients to obtain patient histories using open-ended questioning techniques. Based on my physical exam and history, I create problem lists with associated ranked differentials for working through a problem-oriented approach to diagnosis. I perform abdominal ultrasounds and ultrasound-guided fine needle aspirates with instruction in basic techniques from the staff internists. I critically assess labwork results in conjunction with the presentation of each patient to create either a diagnostic or therapeutic plan based on the diagnosis. In addition, I am receiving further experience in the diagnosis and long-term management of gastrointestinal, respiratory, renal, hematologic, and endocrine diseases such as diabetes mellitus, DKA, hypo- and hyperadrenocorticism, inflammatory bowel disease, chronic kidney disease, IMHA, and inflammatory rhinitis. I assist with endoscopic procedures, including upper and lower GI endoscopy, cystoscopy, and rhinoscopy. During these procedures, I obtain biopsies during endoscopic procedures as well as learning basic techniques of instrument handling. Once weekly, I prepare journal or physiology rounds, critically reviewing two recent journal articles or presenting a chapter from Rose Clinical Physiology of Acid-Base and Electrolyte Disorders.
