With over two years of work experience as a medical social worker, I have developed core competencies in end-of-life care, case management, and mental health care. My mission is to provide compassionate and holistic services to individuals and families facing chronic or terminal illnesses, and to empower them to achieve their goals and improve their quality of life. I am currently working as a medical social worker and manager of volunteer services at Gentiva, a leading provider of home health and hospice care.In my current role, I assess the psychosocial needs of patients and families, coordinate and implement care plans, provide counseling and education, and advocate for their rights and preferences. I also manage and train a team of volunteers who assist with various tasks, such as companionship, respite, bereavement, and administrative support. I use my skills in disability case management, support groups, and mental health care to provide effective and evidence-based interventions that enhance the well-being and dignity of my clients. I also collaborate with an interdisciplinary team of nurses, physicians, therapists, and chaplains to ensure coordinated and comprehensive care.
