Reporting directly to the President of Granger Medical Clinic, the Director of Population Health is responsible for the leadership and strategic direction in the design, development, implementation, and operations of the Population Health Department initiatives. This position will be responsible for operational and strategic goals of the Population Health Department. Essential Functions and Duties• Establishes a population health strategy that supports and aligns with Granger’s organizational strategy, mission, values, and goals. • Provide direct oversight over the Population Health Department.• Develops strategic and operational improvement for the Population Health Department. • Ensures effective processes are in place to foster open lines of communications with leadership, physicians, and staff relating to population health. Ensures information is communicated vertically and laterally to other departments as needed, and keeps leadership informed of any issues.• Establishes and maintains operational budget for the Population Health Department, including developing and maintaining appropriate staffing levels. Monitors the budget to ensure the department is utilizing resources in a fiscally responsible manner. • Assist Accounting Department with provider breakouts for bonus programs.• Train and mentor the Population Health Managers.• Promotes Population Health Department initiatives through education of staff and providers.• In conjunction with the Population Health Managers, develop new care programs. • Provides support and leadership for key meetings including PACU Board, and Payer meetings. • Relationship liaison with payers, including assisting with negotiations/contracting. • Leads, assigns, and participates in special projects or assignments, as needed.

Responsible for operational and strategic goals of the Population Health Department. Essential duties and responsibilities include the following:• Provide direct oversight over the Population Health Department• Work with the President of Granger Medical Clinic to develop strategic and operational improvement for the Population Health Department • Meet monthly with the payers to discuss performance and metric improvement • Assist Accounting Department with provider breakouts for bonus programs• Train and mentor the Population Health Managers• Provide staff and provider in-services regarding the bonus and care programs, as needed• In conjunction with the Population Health Managers, develop new care programs • Member of the Operations Committee for Physician's Accountable Care of Utah ACO

As the Quality Manager of Granger Medical Clinic, I am responsible for managing and overseeing all of the Medicare Advantage and Quality programs at Granger. As part of the Population Health Team, we look at ways that our providers can improve both clinical and financial outcomes for the clinics.

As the Practice Coordinator for Gaylord Physical Therapy, Orthopedics, and Sports Medicine, I have redesigned the benchmarking process for the outpatient physical therapy department to more accurately asses clinician productivity. Streamlined co-pay audit process to improve front office operations and aided in the registration mapping and process improvement for Gaylord outpatient departments.
