As a Case Planner I am responsible for case intakes, management of cases, and provide services needed such as providing clients with community resouces with in or outside of our agency Safe Space. I coordinate all case management, referral and advocacy services for each family. Provide individual, family and group counseling and coordinate and implement casework conferences, as needed.
