Provide exemplary care within 16 bedded high dependency patients presented with chronic health conditions from intensive care unit or have undergone complicated surgeriesKey Responsibilities• providing care for high dependency patients by managing acute to chronic health issues, assessing and evaluating individual patient requirements• Ensuring the timely and accurate completion of all administration related to patient care, Medication including oral and intravenous medication, documentation• Acting in the best interest of patients while ensuring compliance with trust policies and NMC guidelines• Collaborate closely with multidisciplinary team to ensure appropriate treatment/ care are provided to patients• Promote effective communication with patients and relatives• Providing high standards of patient care ensuring patients with specific care requirements such as dementia and Alzhermia are met• Working in collaboration with other health professionals ( OT, PT) to ensure specific needs of patients with reduce mobility and patient needing package of care are met and ensure a safe discharge • Deliver advice to patients concerning diet and lifestyles commitments to complement the medication required to recovery and signpost patients to appropriate specialists if required
