The Systems Navigator helps people experiencing mental illness build social support and community inclusion while helping them navigate the local mental health, physical health, human services, and criminal justice systems so they may lead healthier, meaningful, and self-directed lives in their home communities. As an unbiased and independent broker of services and supports, the Systems Navigator will help clients identify and successfully access community resources necessary to reach self-defined recovery goals, improve their social determinants of health, and live self-directed lives.  DUTIES:Perform prescreening evaluations and recommend diagnosesIdentify candidates for OPC and AOTBe a direct liaison and advocate between the chancery court, state hospital providers, evaluating physicians, crisis centers of Region II, community supports, and treatment team membersCoordinate care for people on outpatient commitment orders with the courtWork with the Chancery Courts of Region II to establish AOT programsEstablish a trusting relationship with clients by initiating and maintaining regular client contact and providing emotional supportRegularly help clients reflect on their progress toward their goalsProvide information, referral and support in locating required services and supportsPatient GPRA data collection for SAMHSA grant reportingHelp clients navigate complex systems, including but not limited to, the mental health care, physical health care, human services, social services, and criminal justice systems with the end goal of assisting clients to build and maintain self-directed lives in their home communities.

Located in the Mississippi Delta, this position primarily involved facilitation of 6 projects. Helped community members build understanding and social capital to support healthier behaviors and lifestyle choices. Developed evaluation protocols and worked with stakeholders, including schools and the Charleston Clinic to establish baseline measures. Coordinated outreach programs. Taught community education courses. Presented programs and outcomes to stakeholders and at various conferences. Wrote grants to help sustain programming. Facilitated grant programming and organizational strategic planning. Worked with the City of Charleston to modify and pass policies related to resident health. Collaborated with the University of Mississippi Center for Population Studies on program evaluation and research.
